Your Respiratory Navigator Software


Nexus is a family of cloud-based, respiratory management software that empowers HMEs to manage their existing populations or enter into value-based care contracts, and profitable new partnerships with sub-acute providers, while increasing referrals.


At the heart of Nexus is an individualized Plan of Care supported by an evidence-based Clinical Response Protocol. Nexus allows HMEs to continually assess patient progress, intervene with clinical protocols, instruct to improve patient self-management, and apply powerful analytics to report on population health management goals.

Assess



Validated assessments measure patient activity, dyspnea control, proper medication usage, and ability to manage their symptoms. 



INTERVENE



Standard care protocols and guidelines are combined with custom care algorithms to create an individualized Plan of Care supported by a Clinical Response Protocol.


Respiratory Therapists provide intervention and instruction tailored to each patient based on standing orders and individual progress.



INSTRUCT



Workbook content corresponds to key interventions in the Plan of Care, allowing the patient to track their progress towards achieving competency badges related to self-management of their respiratory condition.



REPORT
